- 免疫チェックポイント阻害剤 (ICI) は有効であることが示されていますが,化学療法との組み合わせは,特定の患者集団でのさらなる調査を必要とします.
研究 の 目的:
- PD- L1発現が50%以上で標的となる変異がないmNSCLC患者の第一線治療において,ICI単独療法とICI加化学療法 (CIT) の実効性を比較する.
主な方法:
- 311人のmNSCLC患者を特定するために,実際のデータベースが使用されました.
- 患者は第一線ICI単独治療またはCITを受けた.
- 次の治療までの時間 (rwTTNT),無進行生存期 (rwPFS) および全生存期 (rwOS) を含むアウトカムは,調整された比較のために治療重み付けの逆確率 (IPTW) を使用して分析されました.
主要な成果:
- CIT群 (n=133) は,ICI単独治療群 (n=178) (7. 63ヶ月) と比較して,調整された平均 rwTTNT (11. 31ヶ月) を示した.
- しかし,調整された中位 rwPFS (CITでは8. 78ヶ月,ICIでは5. 56ヶ月) や調整された中位 rwOS (CITでは23. 08ヶ月,ICIでは20. 28ヶ月) には大きな違いは見られなかった.
結論:
- この患者群では,ICI単独治療と比較して,CITは次の治療までの時間が長くなったが,進行性のない生存率または全生存率の改善にはつながらなかった.
- 化学療法とICIの併用は,PD- L1発現が高く,標的となる原動因変異がない第一線mNSCLC患者の生存を向上させるようには見えない.
