背景:

カルシヌーリンの阻害剤 (CNIs) はGvHDの予防に標準的に用いられるが,すべての患者が耐えるわけではないので,代替策が必要である.

標準的なCNI治療に耐えられない患者で,この治療法を評価する.

シロリムスと低曝露CNIを投与された16人のallo- HSCT受容者のGvHDの予防のために遡及的分析.
GvHDの発生率,生存率,再発率,感染症合併症,免疫再構成の評価

主要な成果:

急性GvHDの発生率は低い (6. 3%,グレード2,0%グレード3- 4) で,慢性GvHDは観察されなかった.
平均271日間の追跡期間で,再発しない死亡率と3回の再発率 (18. 7%) が良好でした.
治療法は良好で,感染は治癒し,早期のT細胞,B細胞,NK細胞再構成は維持された.

結論:

シロリムスは,低曝露のCNIと併用され,CNIに不耐性のあるallo- HSCT受容者にとって実現可能で有望な代替GvHDの予防策である.
これらの発見を確認するために,より大きな前向きな研究でのさらなる検証が推奨されます.
