糖尿病の高齢者の転倒リスク要因およびその他の老年症候群:多学科転倒相談の経験

目的:本研究の目的は、転倒リスク評価のための多職種連携コンサルテーションを受診した患者における糖尿病の有病率を評価し、糖尿病を有する患者と有さない患者の間で、転倒リスク因子および他の老年症候群の有病率を比較することである。材料および方法:フランスのリール大学病院老年科にて、単一施設でのレトロスペクティブコホート研究が実施されました。組み入れ基準は、2005年1月2日から2015年1月2日の間に、転倒リスク評価のために受診した65歳以上のすべての患者としました。患者個別の転倒リスク評価を確立するため、包括的な多職種による臨床評価が行われました。結果:1,520名の患者が組み入れられた。平均年齢は81.4 ± 6.4歳であり、72.2%が女性、20%に糖尿病があった。糖尿病患者は非糖尿病患者よりも若かった(平均年齢:79.4 ± 6.1歳 vs. 81.9 ± 6.4歳。p< 0.001)であり、過去6か月間に少なくとも2回の転倒を経験している可能性が高いことが示されました(65.5% vs. 56.2%;p= 0.004) では、バランスおよび歩行障害がより多く認められた(それぞれ 77.4% 対 69.7%、p= 0.009であり、88%対82%であった。p= 0.012)であり、糖尿病のない患者よりも認知機能の低下、泌尿器疾患、機能的依存、および多剤併用が多い結果となった(p(すべて p < 0.0001)。結論:糖尿病患者は、非糖尿病患者と比較して、若年であっても老年症候群や併存疾患が多く、有害事象のリスクが高くなります。したがって、糖尿病患者をケアするすべての医療従事者は、転倒やその他の老年症候群の予防に留意すべきです。
