- 基礎認知状態が,CHEIが白質の整体性に影響するかどうかを判断する.

- MCIの被験者における白質の整合性 (微分アニソトロピーと平均拡散性) を分析するために,拡散テンサ画像 (DTI) を2年間使用した.
- この研究では,CHEI (ドネペジルまたはリバスティグミン) で治療された46人のMCI被験者と,未治療のMCI被験者62人を対象とした.
- 統計モデルでは,薬剤,時間,および基礎認知状態 (ADAS- Cog13) の間の相互作用を,共変数で調整した.

- すべてのグループでシングラムバンドル (CGC) の進行性退化が観察されました.
- 微小構造的衰退 (FA損失の減少とMDの増加) が特定の経路 (CGC,EC,PTR) で遅いことが示された.
- 薬剤の作用は,未切除のファシキュルス (UNC) に見られませんでした.
結論:
- ChEIは,MCIの白質を,特に初期段階において,経路特異的な,段階依存の保護を提供します.
- 認知機能が著しく低下する前に ChEI 治療を開始することは,白質の整合性を保つのに不可欠です.
- ステージに合わせた介入は,白質の健康を維持するためにプロドラムアルツハイマー病に必要なものです.
