病院に入院した高齢者の弱弱と認知障害:臨床および検査パラメータとの関連

背景 フレイル症候群(FS)および認知機能障害は、老年医学における主要な懸念事項であり、特に入院中の高齢者に多く見られる。本研究の主な目的は、入院中の高齢患者におけるFSと認知機能障害の併発を評価することであった。さらに、これらの状態と、機能的状態、転倒リスク、感覚欠損、抑うつ症状、および選択された予後判定用の臨床検査パラメータとの関係を分析した。研究仮説は、フレイルの重症度が認知機能障害の悪化と相関しているというものであった。 材料および方法 本レトロスペクティブ研究では、2022年1月1日から12月31日の間にワルシャワ医科大学臨床センター(UCK WUM)の内科・循環器内科クリニックの内科・老年病棟に連続して入院した患者265名の診療録を分析した。入院中、すべての患者に対して包括的老年学的評価(CGA)を実施し、以下のツールを用いた。Katz日常生活動作(ADL)自立度指標、Lawton手段的日常生活動作(IADL)尺度、Barthel Index、Short Tinetti Test、Norton Scale、Mini-Mental State Examination(MMSE)、および15項目高齢者抑うつ尺度(GDS-15)である。フレイルの評価にはClinical Frailty Scale(CFS)を用いた。臨床検査では、血清アルブミン、ヘモグロビン、リンパ球、ナトリウム、および甲状腺刺激ホルモン(TSH)の値を測定した。 結果 大多数の患者(64.77%)にフレイルの徴候が認められ、21.59%がプレフレイル、13.63%が非フレイルに分類された。フレイルの重症度は、認知機能障害の悪化と相関していた。CFSスコアと、年齢(P<0.001)、アルブミン値(P<0.001)、ヘモグロビン値(P=0.002)、リンパ球数(P=0.292)、およびMMSEスコア(P<0.001)などの臨床パラメータとの間に統計的に有意な関連が観察された。機能評価ツール(ADL、IADL、Norton Scale、Barthel Index、Tinetti Test)のスコアは、CFSおよびMMSEの結果の両方と有意に相関していた(P<0.05)。 結論 入院中の高齢患者において、フレイルの重症度は認知機能の低下および機能的パフォーマンスの低下と関連している。アルブミンおよびリンパ球の低値は、より進行したフレイルおよびより重度の認知機能障害と関連していた。
