背景/目的:65歳以上の成人において、fibrosis-4 (FIB-4) がダイナペニアおよび機能的状態と関連しているか、また老年医学的なスクリーニングツールとしての価値があるかを評価しました。方法この横断研究(2023年7月〜2025年7月)では、65歳以上の外来患者537名を対象に評価を行いました。FIB-4は標準的な計算式を用いて算出し、65歳以上の症例に対しては2.0以上を高リスクの閾値として適用しました。参加者をFIB-4低値群と高値群に分けて比較しました。機能状態は、基本的日常生活動作(ADL)、手段的ADL、および歩行・バランステストにより評価しました。ダイナペニアは筋力低下と定義しました。併存疾患、老年症候群、および臨床検査所見を記録しました。結果:高FIB-4群は、高齢であり(78.6 ± 6.0歳 vs. 75.6 ± 5.8歳)、女性の割合が低く、BMIが低かった。POMAおよび手段的ADLスコアは高FIB-4群で低かった一方、基本的ADLおよびTUG所要時間に有意差は認められなかった。握力低下は、高FIB-4でより頻繁に見られた。性別層別解析において、女性では、FIB-4は年齢およびアスパラギン酸アミノトランスフェラーゼ(AST)と正の相関を示し、血小板数、白血球数、握力、POMA、および手段的ADLと負の相関を示した。回帰分析では、握力低下が高FIB-4スコアと関連していたが、高齢女性においては、交絡因子にかかわらずこの関連性は消失した。ダイナペニア群において、BMIの低さと白血球数の多さは、高FIB-4スコアのリスク因子であった。結論:FIB-4は肝線維化のリスクだけでなく、高齢者におけるダイナペニアや機能低下に関連した老年学的脆弱性をも反映しています。年齢調整後のカットオフ値を用いることで、FIB-4は栄養評価や身体機能評価と併せて、実用的な早期警戒スクリーニングとして活用できる可能性があります。
