地上からの転落による環椎および軸椎の同時骨折後の死亡率

目的:
環椎および軸椎の同時骨折患者における高齢および死亡率の高さが、意思決定を困難にしています。本研究の目的は、この患者群における死亡の臨床的および人口統計学的予測因子を特定することでした。
方法:
本レトロスペクティブ・コホート研究では、2012年から2022年までに単一施設で治療を受けた、低所転落による環椎および軸椎の同時骨折患者すべてを対象とした。死亡率の予測因子を特定するために、多変量解析を用いた。
結果:
転倒によりC1およびC2を同時に骨折した患者83名(年齢中央値83歳[四分位範囲 77〜89歳])を対象とした。コホートは女性の割合が高く(61.4%)、重い併存疾患を抱えていた(チャールソン併存疾患指数5以上が54.2%)。転倒の大部分は軽微な外傷であり、患者の57.7%に他の外傷は認められなかった一方、3.7%に重症外傷(損傷重症度スコア 15以上)が認められた。全死亡率は、30日時点で11.4%、90日時点で17.3%、6か月時点で23.5%、12か月時点で28.4%、18か月時点で38.7%、24か月時点で40.7%であり、線形の傾向を示した(R2 = 0.9520, p = 0.0009)。Cox比例ハザード分析において、死亡は高齢(HR 1.048, p = 0.0420)、男性(HR 4.554, p = 0.0009)、および認知症(HR 5.419, p = 0.0011)と関連していた。手術は死亡率に影響を及ぼさなかった(p = 0.8025)。認知症を有する患者は、早期死亡率が高かったが(30日時点で40.0% vs. 4.7%, p = 0.0011)、時間の経過とともに認知症のない患者の死亡率へと収束した。男女の30日時点での死亡率は同様であったが、時間の経過とともに乖離した(p = 0.0460)。男性かつ認知症を合併している群(n = 10)では、2年までの死亡に対する陽性的中率が100%であった(p = 0.0039)。本コホートで観察された生存率は、保険数理上の期待生存率よりも低かった(p = 0.0202)。
結論:
転落により環椎と軸椎を同時に骨折した患者は、一般的に重度の併存疾患を有し、その他の随伴損傷は軽微な80代の女性であった。このコホートにおける死亡率は高く、観察生存率は保険数理上の期待生存率を下回っていた。手術の有無と死亡率に関連は見られなかった。一方で、認知症と男性であることの組み合わせは死亡率と関連していた。認知症の有無による死亡率の差は時間の経過とともに縮小し、男女間の死亡率の差は時間の経過とともに拡大した。
