- 2群の比較:EVAR既往のある患者(rEVAR)と、大動脈への介入既往のない患者(rWPT)。

- 203例のAAA破裂患者を対象に研究を実施。19.7%がEVAR既往あり。
- rEVAR群とrWPT群の間で、来院時の血行動態に有意差は認められなかった。
- 全体の死亡率は40.4%であった。rEVARの死亡率(37.5%)はrWPT(41.1%)と有意差はなかった。
- 年齢、血行動態ショック、開腹手術が死亡率の有意な予測因子であった。
結論:
- EVAR後AAA破裂患者は、新規破裂患者と同様の血行動態不安定性を示す。
- EVAR後AAA破裂の死亡率は高く、新規破裂と同等である。
- 年齢、初期の血行動態ショック、開腹手術を受けることが、AAA破裂患者の死亡率の重要な危険因子である。
