主な方法:

  • リンクされたメディケア請求データを用いた後ろ向きコホート研究。2020年1月~2021年6月に、66歳以上の脳卒中前認知症のないAIS患者を対象とした。退院後12ヶ月間追跡し、ICD-10コードを用いて新規認知症を評価した。

主要な成果:

  • 解析対象は103,378人のAIS患者。COVID-19罹患は1.4%、細菌感染症罹患は5.9%。脳卒中後の認知症発生率:COVID-19群0.011、細菌感染症群0.011、感染なし群0.010。細菌感染症は認知症リスクを有意に増加させた(aRR: 1.13)。COVID-19は同様の傾向を示したが、有意ではなかった(aRR: 1.14)。

結論:

  • 細菌感染症またはCOVID-19を有するAIS患者は、脳卒中後認知症の発生率が高かった。COVID-19感染は、脳卒中後の認知症リスクの上昇に関連している可能性がある。
