  • 特定の変異とAPOE ε4がADADのリスクのある個人の認知成績に影響を与えるかどうかを判断すること。

主な方法:

  • メキシコ系メスティーソ81名を対象に、遺伝子解析(APPV717I/PSEN1A431E変異、APOEジェノタイピング)および神経心理学的評価(CERAD-MX)を実施した。
  • 参加者は変異保有者(C)と非保有者(NC)に分類され、さらにAPOE ε4の状態(ε4+/ε4-)によって層別化された。
  • 認知成績は、12のCERAD-MXタスクにおける正規化zスコアを使用して評価され、群間差はノンパラメトリック検定を用いて分析された。

主要な成果:

  • 変異保有者(C)は、非保有者(NC)と比較して、視覚記憶課題(構成描写-想起、レイ・オーステリット複雑図-想起)において有意に低い成績を示した。
  • 変異保有者の中でも、APOE ε4アレルを持たない者(Cε4-)は、APOE ε4アレルを持たない非保有者(NCε4-)と比較して、レイ・オーステリット複雑図-想起成績が悪かった。

結論:

  • ADADの前臨床段階では、早期の視覚記憶障害が検出可能である。
  • APOE ε4アレルは、このADADコホートにおいて記憶成績に有害な影響を示さなかった。
  • より大規模なサンプルサイズでのさらなる研究が必要であり、発見を確認し、他の認知領域やAPOE祖先などの影響要因を調査する必要がある。
