研究 の 目的:

  • 銀好性顆粒病(AGD)と認知症の臨床病理学的関連を調査すること。AGDとパーキンソニズムとの関連を明らかにすること。サイトウ病期が認知症の重症度に及ぼす影響を分析すること。

主な方法:

  • 高齢者脳バンク(BBAR)コホート(2012-2022)からの452例の剖検症例の後ろ向き分析。AGDの有無に関連する年齢、サイトウ病期、CDRスコアの評価。詳細な臨床的特徴の検討のために、純粋AGDと認知症の症例(CDR≧1、Braak病期≦2、BBARレビー小体病期≦1)を選択。

主要な成果:

  • AGDは50.2%(227/452)の症例で認められ、有病率とサイトウ病期は加齢とともに増加しました。AGD陽性症例は、AGD陰性症例(27.1%、0.8 ± 1.2)と比較して、認知症(46.7%)およびCDRスコア(1.2 ± 1.2)の有意に高い割合を示しました。サイトウ病期IIIの症例は、最も高い認知症の割合(77.6%)とCDRスコア(1.7 ± 1.0)を示しました。純粋AGDによる認知症の20例中6例はパーキンソニズムを呈しました。

結論:

  • 銀好性顆粒病(AGD)は認知症と強く関連しており、特に進行したサイトウ病期において顕著です。パーキンソニズム、特に姿勢不安定性は、AGDの臨床的症状である可能性があります。これらの所見は、高齢者集団におけるAGDが認知機能および運動機能に及ぼす重大な影響を強調しています。
