1型糖尿病患者における低血糖曝露と症状認識の関係における内受容感覚の調整効果

- 1型糖尿病成人(N=717)が、内受容感覚(MAIA-2)および低血糖症状認識のための検証済みアンケートに回答しました。低血糖曝露(30日間の60 mg/dL未満の時間率)を定量化するために、30日間の連続血糖モニタリングデータを収集しました。比例オッズロジスティック回帰により、低血糖曝露と症状レベルとの関連におけるMAIA-2注意調節スコアの調節効果を分析しました。

- 低血糖曝露の増加は、自律神経症状認識レベルの低下と有意に関連していました(OR 0.45、P < 0.001)。内受容感覚(MAIA-2注意調節で測定)は、症状レベルと直接相関しませんでした(OR 0.93)。より高い内受容感覚は、症状認識に対する低血糖曝露の悪影響を有意に減衰させました(OR 1.14、P < 0.05)。
結論:
- 内受容感覚は、低血糖曝露と低血糖症状を認識する能力との関連を調節する上で重要な役割を果たします。将来の研究では、1型糖尿病患者の低血糖認識を回復させる可能性のある内受容感覚を高めることを目的とした介入を探求する必要があります。
