- 患者は全身性掻痒、疲労、浮腫、汎血球減少症を呈した。CT検査でリンパ節腫脹と脾腫が明らかになり、骨髄生検で慢性リンパ性白血病/小リンパ球性リンパ腫(CLL/SLL)が確認された。ESRDと活動性悪性腫瘍のため、患者は標準的ながん治療や腎移植の適応がなかった。
結論:
- 透析患者、特にCKDの原因が不明で血液学的異常がある患者における持続的な掻痒は、潜在的な悪性腫瘍の再評価を必要とする。傍腫瘍性掻痒はがんの早期指標となり、リスクのある集団における早期診断を可能にする。
