- 133個の大腸病変(腺腫78個、過形成ポリープ35個、無茎性鋸歯状病変20個)を検査した。
- 酸素飽和度イメージングを用いて、病変のStO2および周囲粘膜からのStO2差を評価した。
- HIF1αの発現について免疫組織化学を行った。
主要な成果:
- 全ての病変タイプにおいて、周囲粘膜と比較して平均StO2は有意に低かった。
- 腺腫は、過形成ポリープおよび無茎性鋸歯状病変と比較して、有意に大きなStO2差を示した。
- HIF1αの発現は、過形成ポリープおよび無茎性鋸歯状病変よりも腺腫で有意に高く、腺腫ではStO2とHIF1αの間に逆相関が見られた。
結論:
- 大腸腺腫は、他のポリープと比較して有意に低いStO2を示し、より大きな低酸素症を示唆している。
- 粘膜StO2の酸素飽和度イメージング内視鏡評価は、攻撃的な大腸腫瘍性病変の鑑別に役立つ可能性がある。
