研究 の 目的:

  • 畳み込みニューラルネットワーク(CNN)を利用したソフトウェアアプリケーションの効果を評価し、下部消化管処置の質を向上させることを目的としました。多施設共同無作為化比較試験において、ソフトウェア駆動の下部消化管処置アプローチと標準治療を比較することを目的としました。直腸排出液画像のAIベースのフィードバックが結腸内視鏡検査の準備を強化するかどうかを評価することを目的としました。

主な方法:

  • 774人の患者が参加した多施設共同無作為化比較試験。患者は、標準治療またはCNNベースのフィードバックを備えたWebアプリケーションを使用した介入群に割り当てられました。介入群は、AI分析のために直腸排出液の画像を提出し、準備の適切性に関するガイダンスを受けました。

主要な成果:

  • 介入群は、全体的な下部消化管処置の質において統計的に有意な改善を示しました(91% vs. 84.2%、P=0.005)。右側および左側結腸の両セグメントで、介入群においてより優れた処置が観察されました。プロトコル別分析により、介入群では良好な処置(BBPS >7)の達成を含む、統計的に有意に良好な下部消化管処置が確認されました。

結論:

  • 直腸排出液画像のCNN分析を活用するソフトウェアアプリケーションは、外来患者の下部消化管処置の質を大幅に向上させます。このAI駆動アプローチは、結腸内視鏡検査前の下部消化管処置を最適化するための有望なツールを提供します。本研究は、患者ガイドの下部消化管処置プロトコルにAIを統合することの臨床的有用性を示しています。
