  • 交差線維を考慮した、より精密なWM解析のために高度なフィクセルベース解析(FBA)技術を利用すること。

主な方法:

  • DCDの成人16名と健常者13名(18〜35歳)が運動評価(MABC-2)およびMRIスキャン(T1強調画像および拡散強調画像)を受けた。
  • 主要な運動路(CST、小脳脚、SLF)のWM組織化を解析するためにフィクセルベース解析(FBA)が用いられた。
  • 運動関連領域(運動皮質、大脳基底核、小脳、DLPFC)のGM容積が評価された。

主要な成果:

  • DCDの成人と思春期健常者との間で、GMの容積に有意な差は検出されませんでした。
  • 主要な運動路におけるWMの組織化にグループ間で有意な差は見られませんでした。
  • DCDの成人において、WMの組織化と運動パフォーマンススコアとの間に有意な相関は観察されませんでした。
  • ベイズ解析により、グループ差の不在が支持され、左小脳脚(MCP)においては、帰無仮説に対して中程度の証拠が得られました。

結論:

  • 本研究では、FBAを用いたDCDの成人と思春期健常者との間に、有意な構造的脳差(GM容積またはWM組織化)は見られなかった。
  • 本研究の結果は、成人のDCDの主な原因が広範な構造的変化ではない可能性、またはこのサンプルサイズでは検出できないほど効果が小さい可能性を示唆している。
  • これらの所見を確実に確認し、成人DCDにおける潜在的な微妙な神経学的変化を調査するためには、より大規模なサンプルサイズが必要である。
