- ICMを受けた840人のCSまたは一過性脳虚血発作患者の後ろ向き解析。追跡期間の中央値は990日であった。
主要な成果:
- 心房細動(AF)は235人の患者(27.9%)で検出された。再発性脳卒中は112人の患者で発生した。これらのうち70.5%は、AFが検出されなかった患者で発生した。AFが検出された患者では脳卒中再発が増加し(調整後ハザード比:2.73)、CHA2DS2-VAScスコアが4以上および男性も同様であった。
結論:
- CS患者の相当数がICMでAFを検出したにもかかわらず、再発性脳卒中のほとんどは他の原因による可能性がある。CS二次予防のためのICMのルーチン使用は、ランダム化試験によるさらなる調査が必要である。
