- 麻酔科学会パフォーマンスステータス(ASA-PS)≧IIIは、生存率低下の唯一の独立した予測因子であった(HR 3.9)。
結論:
- 内視鏡的粘膜下層剥離術(ESD)は、西洋の文脈におけるEGCを有する80歳以上の患者に無病生存期間の利点を提供する。
- 80歳以上の患者における長期生存率の低下は、主に併存疾患に起因しており、処置前のリスク評価の重要な役割を強調している。
- ESDは安全な手技であるが、処置周りの死亡率の増加のため、重度の併存疾患(ASA-PS≧IV)を有する80歳以上の患者の適応の慎重な評価が不可欠である。
