  • L3椎骨レベルでTPAを測定し、TPA/正常TPA比を算出した。
  • TPA比データにカイ二乗検定およびROC曲線分析を適用した。

  • 軽症膵炎群と中等症から重症膵炎群の間で、TPA比に有意な差が認められた。
  • ROC分析により、TPA/正常TPA比の最適な閾値は1.056であることが特定された。
  • この閾値は、進行予測において62.2%の感度と80%の特異度(AUC=0.761)を示した。

結論:

  • TPAの減少は、急性膵炎が中等症または重症に進行する有意な危険因子である。
  • TPA測定は、急性膵炎における早期リスク層別化のための潜在的なツールを提供する。
  • この所見は、リスクのある患者の転帰を改善するための積極的な臨床介入を支持する。
