主な方法:

  • SARS-CoV-2患者4587人を対象に18ヶ月間追跡した前向き多国籍コホート研究(ORCHESTRA)。
  • 急性感染中および6ヶ月、12ヶ月、18ヶ月で収集された血液検査の多変量解析。
  • 主成分分析を用いた4つのPCC表現型(RESc、CPc、CFc、NSc)の同定。

主要な成果:

  • PCC、特に呼吸器症状を呈する表現型(RESc)の患者は、対照群と比較して12ヶ月および18ヶ月で有意に高いC反応性タンパク質(CRP)レベルを示した。
  • PCCおよびRESc患者のCRPレベルは一貫して3 mg/Lを超え、低炎症を示した。
  • 急性COVID-19の重症度は、フォローアップ中のCRP、フェリチン、LDHの上昇と相関した。初期の生化学的異常はPCCの発症を予測しなかった。

結論:

  • 持続的な低炎症と一致する血液検査の異常は、RESc表現型のPCCにおいて最大18ヶ月まで検出可能である。
  • これらの所見は、PCCの病態における長期的な炎症の役割を支持する。
  • RESc患者を対象とした抗炎症療法および症状特異的介入を標的とした臨床試験が推奨される。
