- 多変量ロジスティック回帰を用いて重み付き4点スコアリングモデルを開発し、ROC解析によってその性能を評価した。

- PR陰性、塩基性様マーカー(EGFRまたはCK5/6)陽性、Ki-67≧50%がpCRの独立した予測因子であった。
- スコアリングモデルはpCRに対して良好な識別能を示した(AUC = 0.754)。pCR率は、低リスク群4.9%、中間リスク群10.7%、高リスク群36.2%であった。
- 高スコア群のpCRは無病生存期間および遠隔転移フリー生存期間を著しく改善したが、低・中間リスク群では有意な生存期間の差は認められなかった。
結論:
- 免疫組織化学ベースのモデルは、pCRを効果的に予測し、NACの恩恵を受けるER陽性/HER2陰性乳がんサブグループを特定する。
- 高スコアの患者はNACから恩恵を受ける可能性がある一方、低・中間スコアの患者は手術と内分泌療法で管理するのが最善かもしれない。
- このモデルは、ER陽性/HER2陰性乳がんにおけるNACに関する個別化された治療決定を支持する。
