- 7件の研究すべてで使用されたスパイロメトリーは、亜臨床的な肺の変化を検出できなかった。急性EC曝露は、スパイロメトリーパラメータ(FEV₁、FVC、PEF、MEF₇₅)およびFeNOを低下させ、呼気温度(EBT)を上昇させ、換気灌流(V/Q)MRI所見を変化させた。慢性EC曝露は、FEV₁、PEF、FEV₁/FVC、FEF25-75%を低下させ、カルボキシヘモグロビン(HbCO)を増加させた。EVALI後の肺機能は、EC中止により改善し、DLCOの増加を示した。
結論:
- スパイロメトリーは第一線のツールであるが、若年EC使用者における早期の肺機能障害を見逃すことが多い。FeNO、DLCO、EBT、MRI、HbCOなどの新たな手法は、スパイロメトリーの限界を補完する。長期的な呼吸器系への影響の早期発見と予防のためには、曝露状況に合わせた多面的な評価が推奨される。
