Shiga toxin 1 (Stx1) reduces the risk of hemolytic uremic syndrome (HUS) in children infected with Shiga toxin-producing E. coli (STEC). Stx1 lowers the release of pathogenic extracellular vesicles (EV) containing Shiga toxin 2 (Stx2), preventing HUS development.
背景:
- STEC由来の志賀毒素(Stx)は疾患を引き起こし、下痢から溶血性尿毒症症候群(HUS)へと移行します。
- 小児におけるHUSは、溶血性貧血、血小板減少症、急性腎不全を伴います。
- HUSのリスクは、産生される志賀毒素の種類(Stx1 vs. Stx2)によって異なります。
結論:
- Stx2aとの併用投与は、病原性EV放出を減少させることにより、HUSリスクを軽減します。
- このメカニズムは、Stx1とStx2の両方を産生するSTECに感染した小児におけるHUSの発生率が低いことを説明しています。
- Stx1は、Stx2駆動性EV病原発生を阻害することにより、重度のHUSに対する保護因子として作用します。
