- 2020年から2024年の間にNEC(Bell Stage II/III)と診断された新生児の後ろ向きコホート研究。診断前15日間および診断後4日間の連続HRCiデータを抽出。一般化加法モデル、ウィルコクソン符号順位検定、ロジスティック回帰分析を用いてHRCiの軌跡を解析し、在胎週数および出生体重のZスコアを調整した。
主要な成果:
- HRCiはNEC発症の2週間前から徐々に上昇を示し、診断の3日前には有意な上昇が観察された(p=0.031)。ロジスティック回帰分析により、HRCiは在胎週数や出生体重のZスコアよりも優れた、NEC診断との独立した関連性が確認された(OR=1.28、p<0.001)。患者内モデルの識別能(AUC 0.65-0.66)は中程度であったが、NICU全体の集団(AUC=0.84)と比較すると有意に改善した。
結論:
- 早産児におけるNEC診断に先行してHRCiの上昇傾向が見られ、臨床発症の数日前には有意な変化が生じる。HRCiはNECとの独立した時間的関連性を示し、非侵襲的バイオマーカーとしての可能性を示唆する。NEC予測におけるHRCiの臨床的有用性を検証するためには、対照群を設定した前向き研究がさらに必要である。
