研究 の 目的:
- 食道癌のために食道切除手術を受ける患者のTDOの栄養および腫瘍学的アウトカムを評価する.
主な方法:
- 2016年1月から2024年5月までの82人のTDOと313人の非TDO患者の遡及的分析.
- 評価された実験室のパラメータ,CTによる体構成,栄養指数,全生存期 (OS),再発のない生存期 (RFS).
- 傾向スコアマッチングにより,75人のTDO患者と150人の非TDO患者による比較グループが作成されました.
主要な成果:
- TDOはリンパ節の産量を増加させましたが,TDOでない人と比較してキロトラクスの発生率,栄養状態,体質,OS,またはRFSに影響を与えませんでした.
- ネオアジュヴァント療法 (NAT) のサブグループは,TDO患者で早期の術後,一時的な皮下脂肪の損失を示し,9〜12ヶ月で解決しました.
- NAT患者では,胸管切除は,切除しない患者と比較して,3年間のOS (77.15%対57.25%) とRFS (72.41%対48.08%) の改善と関連していました.
結論:
- エソファゲクトミー中のTDOは,食道がん患者の長期的な栄養および腫瘍学的結果に関して安全です.
- TDOを受けたNAT患者における早期の術後の皮下脂肪減少は一時的なものです.
- NAT患者における胸管切除は,生存結果を向上させる可能性があります.
