研究 の 目的:

  • 完全に合成されたトレーニングパイプラインを使用して、日本語放射線レポートのコンテキスト認識型文分類モデルを開発すること。;モデル開発中に実際の臨床データへの依存を排除すること。;多様な、複数の機関の実際のレポートでモデルの一般化可能性を評価すること。

主な方法:

  • GPT-4.1を使用して3,104件の合成日本語放射線レポートを生成し、GPT-4.1-miniでアノテーションを付けました。;LoRAを使用して、軽量ローカルLLM(Qwen3、Llama 3.2)および日本語テキスト分類モデル(BERT、JMedRoBERTa、ModernBERT)をファインチューニングしました。;J-MIDからの280件の実際のレポートでモデルを検証し、放射線科医がラベルを確認しました。

主要な成果:

  • モデルは合成データで高いパフォーマンスを達成しました(精度:0.938-0.951、マクロF1:0.924-0.940)。;実際のデータではパフォーマンスが低下しました(精度:0.783-0.813、マクロF1:0.761-0.790)が、陽性所見の陽性予測値(PPV_1)は依然として高く(例:0.952)、維持されました。;LLMベースのアプローチは、外部データセットで軽微な解析エラー(0.55%-7.48%)を示しました。

結論:

  • 合成データを使用して日本語放射線レポートのコンテキスト認識型文分類モデルを開発することは可能です。;モデルは、安定したPPV_1によって示されるように、陽性所見を特定するための重要な臨床用語とパターンを正常に捉えました。;この合成データアプローチは、アノテーションの負担とプライバシーリスクを軽減し、医用画像におけるAI開発をサポートします。
