我々は、Scheieの高血圧性眼底変化に基づき、日本高血圧学会(JSH)2025年ガイドラインのリスク層別化表の妥当性を評価することを目的とした。2017年から2024年までに、さいたま赤十字病院の健康診断で眼底検査を受けた者のデータ(N = 6,953)を分析した。JSH 2025年ガイドラインのリスク層別化表に基づき、年齢、性別、併存疾患、既往歴、および健康診断時の血圧(BP)を用いて、リスクを12群に層別化した。本研究の独自性として、BP < 130/80 mmHgの群をさらに詳細に検討し、計15群を分析した。リスク第1カテゴリーかつBP < 130/80 mmHgの群を対照群とした。リスクカテゴリーおよび血圧レベルが上昇するにつれて、Scheieの高血圧性変化の割合は有意に増加した。同様に、Scheieの硬化性変化の割合も有意に増加した。BP < 130/80 mmHgの範囲内においても、リスクカテゴリーが上昇するにつれて眼底変化の割合は有意に増加した。結論として、健康診断受診者を対象とした場合、JSH 2025年ガイドラインのリスク層別化表は、Scheieの高血圧性変化に基づいた妥当性を有していた。さらに、BP < 130/80 mmHgの群をリスク層別化表に加えることが可能であると考えられた。
