高齢者における肋骨骨折の手術的固定:レトロスペクティブ・コホート研究

高齢者における肋骨骨折の手術的固定術(SSRF)の役割、特に機能回復と長期的な疼痛に関しては、依然として不明確な点が多い。本研究では、暦年齢が術後経過に影響を与えるかどうかを検討し、SSRFを施行した高齢患者における外傷重症度スコアリングシステムの性能を評価した。本レトロスペクティブ・コホート研究には、2012年から2023年の間にSSRFを受けた成人524名が含まれた。患者は非高齢者群(65歳未満、n = 331)と高齢者群(65歳以上、n = 193)に層別化した。人口統計学的特性、外傷特性、周術期アウトカム、疼痛の推移、および機能回復を比較した。術後合併症および慢性疼痛の予測因子は、単変量および多変量ロジスティック回帰を用いて評価した。臨床アウトカムとRibScore、AIS-Thorax、およびBPC18との相関は、スピアマン相関係数を用いて分析した。高齢患者は、低エネルギー転倒による負傷が多く、肺挫傷の発生率が高かったが(74.6%対63.9%)、合併症発生率は若年成人と同程度であった(14.0%対12.7%、p = 0.696)。疼痛の推移は、すべてのフォローアップ期間において同等であった。機能回復は高齢者でより遅く、ベースラインの活動に戻るまでにさらに1か月を要した(中央値 3か月対2か月、p < 0.001)。手術側の肋骨骨折数は、術後合併症の独立した予測因子であったが(OR 1.863, 95% CI 1.396-2.487, p < 0.001)、年齢は独立した予測因子ではなかった。手術までの期間の延長(OR 1.341, p = 0.001)、入院期間の延長(OR 1.059, p = 0.044)、および術後合併症(OR 1.941, p = 0.036)は、慢性疼痛の独立した予測因子であった。AIS-Thoraxは臨床アウトカムと最も強い相関を示したが、3つのスコアリングシステムすべてにおいて、術後疼痛との関連は弱かった。適切に選択された患者において、SSRFは年齢層を問わず同等の安全性と疼痛回復をもたらす。高齢者では機能回復に時間を要するものの、全体的なアウトカムは良好である。これらの知見は、SSRFの決定において、年齢ベースから生理学的状態ベースの意思決定へ移行することを支持している。
