近年来,捷克共和国已建立了急诊科。老年人是这些科室的典型患者。急诊医学的方法基于对症状的评估以及对所需护理优先级的判断。对老年患者的诊疗在诊断和治疗方面均有其特殊性。在对老年患者进行分诊时,若同时评估其认知功能,并使用老年衰弱量表及谵妄筛查工具,分诊将更为准确。全面的老年综合评估耗时较长,因此不适合急诊科环境,但我们必须保留其基本组成部分。治疗策略必须是综合性的,应包括生物、心理、社会因素以及环境风险分析。老年人创伤管理需要采用与常规分诊标准不同的生命功能指标进行评估,同时需考虑药物对机体代偿机制的影响以及低能量创伤机制的风险。创伤治疗必须及时且全面,并确保从重症到普通病房再到康复阶段的连续性照护。对于终末期患者,适宜采取姑息治疗策略。 关键词:急诊科,老年人,分诊,老年综合评估,衰弱,谵妄,创伤管理,姑息治疗
