目的:探讨中国老年人睡眠质量与认知功能之间的双向关系,并进一步利用随机截距交叉滞后面板模型分析该关系中的性别差异。 设计:对一项身体活动集群随机对照试验(“衰老期间保持活跃”)进行二次观察性分析。 地点:中国四川省八个村庄。 研究对象:共511名年龄≥60岁的成年人,应答率为97.3%。 测量方法:采用匹兹堡睡眠质量指数量表评估睡眠质量,认知功能通过电话认知状态访谈量表进行评估。 结果:平均年龄为71.0岁(标准差5.710),男性227名(44.4%)。前一时点的睡眠质量与后一时点的认知功能相关(β = -0.135,[95%CI -0.244 至 -0.026],第2波至第3波;β = -0.108,[95%CI -0.204 至 -0.013],第4波至第5波)。前一时点的认知功能亦与后一时点的睡眠质量相关(β = -0.404,[95%CI -0.566 至 -0.242],第3波至第4波;β = -0.224,[95%CI -0.392 至 -0.055],第4波至第5波)。上述关系仅在女性中具有显著性。 结论:老年人睡眠质量与认知功能之间存在双向关联,尤其在女性中更为明显。未来针对认知功能的干预措施可考虑改善睡眠质量,反之亦然,尤其应关注女性群体。
