背景与目的:尿失禁(UI)在老年人群中是一种常见问题,可能需要尿动力学检查(UDS)以进行准确诊断。然而,这些检查过程可能令人不适且耗时较长,尤其是在老年患者群体中,可能存在某些实际限制。本研究探讨在老年患者群体中进行UDS时,充盈期膀胱压力测定的体位是否影响诊断结果,以及单一体位的一次UDS是否足以提供可靠的诊断,或多种体位是否能带来额外的诊断价值。材料与方法:这是对“Think Dry:老年人尿失禁诊断流程优化研究”(NCT04094753)的二次分析,该研究是一项前瞻性观察性队列研究。每位患者在尿动力学检查(UDS)过程中均接受了坐位和站立位的充盈期 cystometry 检查。最终诊断由转诊泌尿科医师结合坐位组和站立组的结果以及所有可用的临床数据综合确定。随后,由第二位不知分组情况的泌尿科医师独立回顾每项单独的尿动力学检查,并基于单次 UDS 建立诊断。然后采用 Cohen's kappa 系数(κ)评估这些独立诊断与最终诊断之间的一致性。结果: 与最终诊断相比,站立位充盈性膀胱压力测定的尿动力学检查(UDS)结果具有几乎完美的一致性(κ = 0.92),而坐位时仅表现出中等一致性(κ = 0.42)。结论: 采用立位充盈期膀胱压力测定的尿道压力分布测定(UDS)可能足以实现准确诊断,从而有望无需再进行坐位充盈期膀胱压力测定。通过简化诊断流程,该方法可提高诊断效率,减轻患者负担,并优化老年患者医疗资源的利用。
