本病例报告描述了一位70多岁的日本女性,最初被诊断为弥漫性泛细支气管炎(DPB),并接受了长期的大环内酯类药物治疗。患者表现为持续性咳嗽和脓性痰液,胸部CT显示呈小叶中心性磨玻璃影。该患者的临床病程包括以下发现:Pseudomonas aeruginosa并伴有阻塞性功能障碍,符合弥漫性泛细支气管炎(DPB)的表现。然而治疗无效,且肺部磨玻璃影随时间进展加重。随后,在初次发病6年后,患者出现黄甲,诊断修正为黄甲综合征(YNS)。该病例具有重要意义,其呈现的肺小叶中心性磨玻璃影与DPB高度相似,提示YNS中淋巴管功能障碍可能参与了肺小叶中心性磨玻璃影的形成。
