[脊狭窄症:一种普遍的疾病还是仅仅是放射性诊断?]

腰椎管狭窄症主要影响65岁以上老年人,常伴有典型的临床症状,如神经源性间歇性跛行。除罕见的先天性因素外,该病通常继发于退行性改变,如椎间盘突出、小关节退变和韧带肥厚。除典型临床表现外,双平面站立位X线平片和磁共振成像(MRI)具有决定性诊断价值。对于无明显神经功能缺损的患者,应首选保守治疗。当出现保守治疗无法缓解的疼痛、存在神经功能缺损或保守治疗无效时,可考虑手术治疗指征。应尽可能选择微创减压手术。若存在具有临床意义的不稳、狭窄或力线异常,则可能需要行融合术。 关键词:腰椎管狭窄症,神经源性间歇性跛行,退行性改变,椎间盘突出,小关节退变,韧带肥厚,站立位X线,磁共振成像,MRI,保守治疗,微创减压,融合术
