Summary

This study evaluated tuberculosis control activities in Nepal before DOTS strategies. While cure rates were comparable to DOTS-adopting countries, treatment completion saw an increase in defaulters, suggesting room for improvement.

Area of Science:

  • Public Health
  • Infectious Disease Epidemiology
  • Global Health

Context:

  • Pilot tuberculosis control activities were implemented in Lumbini, Rupandehi district, Nepal.
  • The project was a collaboration between the Nepal Anti-Tuberculosis Association (NATA) and the Japan Anti-Tuberculosis Association (JATA).
  • The initiative aimed to achieve high cure rates for new smear-positive pulmonary tuberculosis patients prior to the adoption of DOTS strategies.

Purpose:

  • To assess the effectiveness of pilot tuberculosis control measures.
  • To analyze treatment outcomes for different categories of tuberculosis patients.
  • To establish a baseline for treatment success before implementing Directly Observed Treatment, Short-course (DOTS) strategies.

Summary:

  • Between December 1993 and July 1996, 349 tuberculosis cases were enrolled, categorized into new smear-positive pulmonary TB, other smear-positive pulmonary TB, new smear-negative TB, and other smear-negative TB.

Related Experiment Videos

  • Chemotherapy regimens were standardized for each TB category, with increasing proportions of patients receiving appropriate treatment.
  • Cohort analysis revealed overall cure and treatment completion rates of 74% for new smear-positive pulmonary TB and 77% for smear-negative pulmonary TB, though a rise in defaulters was observed in the third year.
  • Impact:

    • Treatment outcomes demonstrated cure and completion rates comparable to SEARO countries utilizing DOTS strategies.
    • The findings suggest that the implementation of DOTS strategies in 1997 could further enhance treatment outcomes.
    • This study provides valuable data on tuberculosis control effectiveness in a pre-DOTS era, informing future public health interventions.
