Abstract:
Due to documented problems encountered during therapy, the potential for drug-drug interactions, and possible readmissions due to therapy complications, the Ohio State University Department of Pharmacy decided to conduct a drug usage evaluation (DUE) of warfarin sodium. The results of their DUE are presented below.
