Summary

Coronary angioplasty, primarily using balloons, has expanded its applications despite restenosis remaining a challenge. Future trials will compare angioplasty with surgery, but individual patient needs will guide revascularization choices.

Area of Science:

  • Cardiovascular Medicine
  • Interventional Cardiology

Background:

  • Coronary angioplasty has evolved significantly over 16 years.
  • The balloon catheter remains the primary tool, with stents being the main innovation.
  • Operator experience and technological advancements have broadened procedure indications.

Purpose of the Study:

  • To review the advancements and current status of coronary angioplasty.
  • To highlight expanded indications and persistent challenges like restenosis.
  • To contextualize ongoing comparative trials with surgical revascularization.

Main Methods:

  • Review of historical development and current practices in coronary angioplasty.
  • Discussion of expanded indications including multivessel, chronic occlusions, grafts, acute myocardial infarction, and high-risk procedures.
  • Acknowledgement of ongoing trials comparing angioplasty and coronary artery bypass grafting (CABG).

Main Results:

  • Despite new devices, the balloon angioplasty technique remains central.
  • Indications have expanded to complex cases, but single-vessel angioplasty is most common.
  • Restenosis remains a significant unsolved problem following angioplasty.

Conclusions:

  • Coronary angioplasty has broadened its scope, yet restenosis persists.
  • The optimal revascularization strategy for individual patients requires further clarification.
  • Upcoming comparative trials are anticipated but may not provide definitive answers for all patient scenarios.
