Summary

Lymphedema causes leg swelling and skin thickening, which impairs venous return. This venous return impairment can worsen lymphedema, creating a detrimental cycle.

Purpose of the Study:

  • To investigate potential anatomical and functional impairments of venous return in patients with leg lymphedema.
  • To correlate lymphedema severity with venous dynamics.

Main Methods:

  • Utilized Volometry for leg volume assessment.
  • Employed color Duplex Doppler for deep vein patency and skin thickness.
  • Applied Air-plethysmography to measure ambulatory venous pressure, venous volume, venous filling index, and ejection fraction in 41 patients.

Main Results:

  • Lymphedematous legs showed significant increases in volume (126.4%) and skin thickness (156.9%).
  • Ambulatory venous pressure (134%), venous volume (124.5%), and venous filling index (134.5%) were elevated, while ejection fraction decreased (94.9%).
  • Increased leg volume and subcutaneous thickness correlated with impaired venous filling and reduced ejection fraction.

Conclusions:

  • Soft tissue edema in lymphedema progressively impedes venous return.
  • Impaired venous return may exacerbate the progression of lymphedema, suggesting a feedback loop.
