    Summary

Exercise stress tests, like bicycle ergometer or treadmill tests, are effective for diagnosing angina pectoris when resting ECG is inconclusive. These performance tests accurately identify coronary artery disease in 75.5% of patients.

    Area of Science:

    • Cardiology
    • Diagnostic Imaging

    Context:

    • Anamnesis alone is often insufficient for diagnosing angina pectoris.
    • Resting electrocardiograms (ECG) typically show minimal changes in angina pectoris.
    • Complementary diagnostic tests are frequently required.

    Purpose:

    • To evaluate the diagnostic utility of physical activity-influenced ECG in angina pectoris.
    • To compare the efficacy of exercise performance tests with coronarography.
    • To determine the sensitivity and validity of stress testing for coronary artery disease.

    Summary:

    • Exercise performance tests, utilizing bicycle ergometry or treadmills, are valuable when resting ECG is non-diagnostic for angina pectoris.
    • A study of 319 patients showed a 75.5% positive rate for performance tests compared to coronarography.
    • Test sensitivity is influenced by factors like maximal heart rate achievement, stenosis severity, angina type, and resting ECG patterns.

    Impact:

    • Highlights the importance of exercise stress testing in diagnosing angina pectoris.
    • Provides evidence for the effectiveness of performance tests in identifying coronary artery disease.
    • Suggests that maximal heart rate testing offers superior sensitivity compared to pharmacological stress methods.
