Background:

  • Iris-nevus syndrome is a rare condition.
  • Glaucoma can be associated with iris-nevus syndrome.

Observation:

  • A 39-year-old woman with blurred vision and left glaucoma presented with iris atrophy, nodular iris nevus, and a distorted pupil.
  • Specular microscopy showed low endothelial cell density in the left cornea.
  • The iridocorneal angle was closed with peripheral anterior synechia.

Findings:

  • Indocyanine green and fluorescein iris angiography revealed increased iris vascularity and leakage.
  • Histopathology of trabeculectomy tissue showed corneal endothelial cells on the iris surface, increased iris stroma vascularity, and degenerated vessel wall cells.
  • Narrowed Schlemm's canal and closed intertrabecular spaces were observed.
