Summary

Delayed diagnosis of tuberculous peritonitis can be fatal. Prompt diagnosis of ascites, especially peritoneal tuberculosis, requires aggressive workup including laparoscopy or newer tests like adenosine deaminase activity and polymerase chain reaction.

Area of Science:

  • Infectious Diseases
  • Gastroenterology
  • Pulmonology

Background:

  • Tuberculous peritonitis is a serious condition often presenting with non-specific symptoms.
  • Diagnostic delays can lead to severe morbidity and mortality.

Observation:

  • A fatal case of tuberculous peritonitis highlights diagnostic challenges.
  • Laparoscopy is presented as a safe and effective bedside procedure for diagnosing ascites.

Findings:

  • Newer noninvasive tests, including ascites fluid adenosine deaminase activity and polymerase chain reaction, show promise for prompt diagnosis.
  • Aggressive diagnostic workup or empirical antituberculous therapy is recommended for suspected peritoneal tuberculosis.

Implications:
