Summary

Occupational asthma in a metal worker was linked to neutrophilic bronchitis, not the typical eosinophilic type. Symptoms and lung function improved when away from work, confirming the occupational link.

Background:

  • Occupational asthma is a significant workplace health concern.
  • It is often characterized by eosinophilic airway inflammation.
  • Metal working fluids (MWFs) are known respiratory irritants.

Observation:

  • A case study of a 41-year-old woman with asthma symptoms after MWF exposure.
  • Pulmonary function tests confirmed asthma with significant airway obstruction.
  • Induced sputum analysis revealed marked neutrophilia, not eosinophilia.

Findings:

  • Sterile neutrophilic bronchitis correlated with occupational MWF exposure.
  • Airway inflammation and obstruction resolved upon removal from exposure.
  • Re-exposure led to recurrence of neutrophilic bronchitis and asthma symptoms.

Implications:

  • This case suggests occupational asthma can present with neutrophilic bronchitis.
  • It highlights the need to consider MWFs as a cause of non-eosinophilic occupational asthma.
  • Further research into MWF-induced airway inflammation mechanisms is warranted.
