Background:

Ventricular septal defects (VSD) are typically considered non-life-threatening congenital heart conditions.
While many VSDs close spontaneously or are surgically manageable, a subset presents significant risks.

Observation:

Serious arrhythmias affect 16-31% of patients with VSD.
Sudden death occurred in 4.2% of patients in one study, accounting for one-third of deaths in medically managed cases.
Cardiac hypertrophy was a common finding in VSD-associated sudden death cases.

Findings:

Four cases of sudden unexpected death in infants (1 week to 3.5 months) with VSD were observed.
Cardiomegaly was present in all four cases.
Pulmonary arteriolar medial thickening and smooth muscle extension were noted in one infant.

VSDs in infants and young children may be potentially life-threatening malformations.
These findings underscore the necessity for vigilant clinical monitoring of infants with VSD.
