Observation:

  • A 62-year-old patient with rheumatoid arthritis developed severe polymyositis.
  • The patient was treated with tiopronine, a thiol-containing drug.
  • Clinical presentation and pathological findings were characteristic of polymyositis.

Findings:

  • Discontinuation of tiopronine and initiation of methotrexate therapy led to a favorable clinical course.
  • This case highlights a rare but serious adverse effect of tiopronine.
