    Background:

    • Marasmus, a form of severe protein-energy malnutrition, presents with growth retardation and muscle wasting.
    • Assessing nutritional status and treatment response in children with marasmus requires objective biomarkers.

    Purpose of the Study:

    • To compare serum lysosomal hydrolase activities in well-nourished children and those with marasmus.
    • To evaluate the potential of serum hydrolase ratios as indicators of malnutrition and treatment efficacy.

    Main Methods:

    • Serum samples from 19 well-nourished children and 13 marasmic children (5-24 months) were analyzed.
    • Activities of beta-glucuronidase, acid phosphatase, and hexosaminidase were measured using fluorogenic substrates.
    • The ratio of beta-glucuronidase to acid phosphatase was calculated and compared between groups.

    Main Results:

    • Marasmic children showed significantly higher serum beta-glucuronidase (2.3-fold) and lower acid phosphatase (50%) compared to controls.
    • Serum hexosaminidase levels did not differ significantly between the groups.
    • The beta-glucuronidase to acid phosphatase ratio effectively discriminated between normal and malnourished children (p < 0.001).

    Conclusions:

    • The elevated beta-glucuronidase:acid phosphatase ratio in marasmus normalized after nutritional rehabilitation.
    • Serum lysosomal hydrolase determination offers a potentially rapid and sensitive method for evaluating protein-calorie malnutrition and treatment response in children.
