Summary

Prophylactic cerclage significantly increases cervical length in women with an incompetent cervix. This measurable lengthening may improve the success rate of cerclage procedures.

Area of Science:

  • Maternal-Fetal Medicine
  • Surgical Obstetrics

Background:

  • Cervical length is a critical factor in maintaining pregnancy.
  • Cervical incompetence is associated with an increased risk of preterm birth.
  • Cerclage is a surgical procedure to reinforce a weakened cervix.

Purpose of the Study:

  • To evaluate the change in cervical length following cerclage placement.
  • To assess the efficacy of transvaginal ultrasound in measuring cervical length changes post-cerclage.

Main Methods:

  • Prospective, observational study design.
  • Transvaginal ultrasound used for cervical length measurement before and after cerclage.
  • McDonald or Shirodkar suture techniques employed.
  • Mean cervical length calculated after excluding the first measurement.

Related Experiment Videos

Main Results:

  • A total of 31 cerclage procedures were performed (21 Shirodkar, 10 McDonald).
  • Mean pre-cerclage cervical length was 2.7 cm (+/- 0.9 cm).
  • Mean post-cerclage cervical length significantly increased to 3.6 cm (+/- 0.9 cm) (P<.001).

Conclusions:

  • Prophylactic cerclage leads to a statistically significant increase in cervical length.
  • The observed cervical lengthening may contribute to the success of cerclage.
  • Further research is required to determine if the degree of lengthening predicts delivery at term.
