[Clinical results of cord blood transplantation]
1Department of Pediatrics, Tokai University School of Medicine.
Insights
Cord blood transplantation shows good results for siblings, comparable to bone marrow transplants. Unrelated transplants and adult cases face challenges, requiring further study for better outcomes.
Area of Science:
- Hematology
- Transplantation Medicine
- Immunology
Context:
- Cord blood stem cell transplantation is a recognized therapy worldwide.
- Over 1000 cord blood transplantations have been performed globally.
- 79 cases have been reported to the Japanese Ministry of Health.
Purpose:
- To evaluate the outcomes of cord blood transplantation.
- To compare sibling versus unrelated donor transplantation results.
- To assess the efficacy of cord blood transplantation in adult patients.
Summary:
- Sibling cord blood transplantation survival rates are comparable to bone marrow transplantation.
- Unrelated cord blood transplantation outcomes are poorer, with lower engraftment and higher mortality.
- Adult cord blood transplantation results are suboptimal, potentially due to advanced disease stages.
Impact:
- Sibling transplants are a viable alternative to bone marrow transplants.
- Unrelated cord blood transplantation requires optimization to improve success rates.
- Further research is needed to enhance adult cord blood transplantation efficacy.
Abstract:
Nearly 1000 cases have received cord blood transplantation in the world since the first successful transplantation by Gluckman. 79 cases have been reported to the Ministry of Health in Japan. Results of sibling transplantation have been good and the survival rates are as good as those in bone marrow transplantation. On the other hands, results of unrelated transplantation seem to be poor due to lower engraftment rate and higher transplant-related mortality. Number of attempts in adults is increasing, but the initial results do not look so good, probably because many transplants were done at the advanced stages, specially in adults. Further analysis and follow-up are needed for more accurate evaluation of this treatment modality.
