Summary

This study reports the first clear case of diffuse large B cell lymphoma (DLBL) expressing the natural killer (NK) cell antigen CD56. This rare finding in a common B-cell lymphoma offers new insights into DLBL heterogeneity.

Background:

  • The natural killer (NK) cell antigen CD56 is rarely expressed in hematological malignancies.
  • While CD56 expression is reported in some T/NK cell lymphomas, multiple myeloma, and acute myeloid leukemia, its presence in B-cell non-Hodgkin's lymphomas (B-NHL) is exceptionally uncommon.
  • Previous reports on CD56 in B-NHL are limited, with only one study noting weak expression in diffuse large B cell lymphoma (DLBL).

Observation:

  • This report details the first documented case of DLBL with clear CD56 expression.
  • Immunophenotypic analysis revealed the tumor cells as CD10+, CD19+, CD20+, CD45RO-, CD3-, and CD56+.
  • Immunohistochemical studies showed no positivity for bcl-2 or TIA-1 in the tumor cells.

Findings:

  • The tumor exhibited monoclonal immunoglobulin heavy chain (IgH) gene rearrangement.
