Summary

GnRH agonist therapy effectively treated sciatic nerve endometriosis, improving pain and motor function in patients. This suggests endometriosis of the sciatic nerve may be more common than previously recognized.

Area of Science:

  • Gynecologic Surgery
  • Neurology
  • Reproductive Medicine

Background:

  • Pelvic endometriosis can cause sciatic nerve damage, presenting as cyclic sciatica.
  • Diagnosis is challenging due to negative imaging findings in some cases.
  • Endometriosis affecting the sciatic nerve is potentially underdiagnosed.

Observation:

  • Three patients with cyclic sciatica and confirmed sciatic nerve damage due to endometriosis were studied.
  • Standard imaging techniques (CT, MRI) did not reveal the sciatic nerve involvement.
  • Patients received monthly GnRH agonist (leuprolide acetate) with transdermal estrogen therapy.

Findings:

  • All patients experienced significant pain reduction.
  • Motor function, including claudication, showed partial improvement.

Related Experiment Videos

  • Clinical response to therapy served as a diagnostic indicator.
  • Implications:

    • GnRH agonist therapy demonstrates efficacy in managing sciatic nerve endometriosis.
    • Clinical response to GnRH analogues can aid in diagnosing elusive sciatic nerve endometriosis.
    • This condition may be more prevalent than current diagnostic methods suggest.
