Background:

Paragonimiasis is a food-borne parasitic lung fluke infection.
Raw freshwater crab consumption is a known risk factor for Paragonimiasis miyazakii.

Observation:

A 77-year-old woman presented with hemoptysis and weight loss following raw crab ingestion.
Initial symptoms included pleural effusion, which resolved with chemotherapy.
Recurrent hemoptysis and a migratory pulmonary nodule were observed, alongside eosinophilia and elevated IgE.

Findings:

Diagnosis confirmed as Paragonimiasis miyazakii infection through serologic tests.
The migratory nature of the pulmonary lesion was a key diagnostic indicator.
Successful treatment with praziquantel was achieved.
