Summary

Most anal fistulae originate from proctodaeal glands and involve only part of the sphincter muscles. Laying open the main track is the best treatment, preserving continence by saving sphincter muscle.

Area of Science:

  • Gastroenterology
  • Colorectal Surgery

Context:

  • Anal fistulae commonly originate from proctodaeal glands, with tracks traversing only portions of the anal sphincter muscles.
  • Understanding the anatomical course of anal fistulae is crucial for effective surgical management.

Purpose:

  • To outline the primary origin of anal fistulae and discuss optimal surgical strategies.
  • To emphasize the importance of preserving anal sphincter function during fistula treatment.

Summary:

  • The majority of anal fistulae stem from proctodaeal glands, typically involving partial sphincter penetration.
  • Surgical treatment, primarily laying open the fistulous track, aims to preserve anorectal continence by minimizing sphincter damage.
  • Alternative methods like exteriorization may be necessary in complex cases but carry higher risks.

Impact:

  • This approach facilitates wound healing and scar integration, maintaining continence in most patients.
  • Highlights the necessity of sphincter preservation for functional outcomes in anal fistula surgery.
  • Informs surgical decision-making for complex or iatrogenic fistulae, balancing efficacy with functional preservation.
